Complete your health questionnaire
If you haven't already, fill out the prequalification form so our physician has your diagnosis, medications, and symptom history in front of them during the consultation. This shaves hours off the turnaround.

Physician review & documentation
A U.S.-licensed physician or optometrist reviews your records and, where clinically appropriate, documents your medical necessity on your state's exemption form. Turnaround is typically 24–48 hours after we have your health questionnaire on file.

Delivery & DMV/state submission
You receive the completed form and any supporting physician letter by email and, when your state requires, in the mail. We include state-specific instructions for submitting to your DMV or state police. Then you schedule your tint installer with confidence.
